The form route
Complete the relevant sections
The official form asks for sections 1, 2 and 4, then the selected system in section 5.
Obtain CPAM certification
The French CPAM completes section 6 before final submission to the authority in the canton of employment.
Meet the deadline
Submission is generally required within three months of starting Swiss work or a recognised new triggering event.
From starting work to proof of choice
The employment canton is responsible; this is not automatically Geneva.
Download the FOPH form
Confirm that the event gives rise to an option right.
Record and sign the choice
Complete personal, employment and selected-system fields.
Get the CPAM section completed
Ask CPAM to certify section 6 and retain a dated copy.
File with the canton
Send the completed file to the employment canton within the applicable period.
